I find that pod solves many of the problems. Document and comments
intermixed sensibly, thus documentation and code are in the same
place. Useful output format (man) which can be HTMLised easily and a
reader which is good, simple and present everywhere a user might choose
to log in (viz. $PAGER).

I don't know how well it'd work with a language like C though.

Of course for detailed documentation you probably want a web site
distributed with the application, perhaps in a -doc package.
