Huh, ok. I've only ever had to put in the same details you use when connecting
using a USB modem. ie. VCI/VPI (usually the defaults work unless you're using
a US import), username, password, and maybe "Phone Number". Everything else
autoconfigures via DHCP.
Figuring out the correct spelling of username is sometimes non-obvious, and
many routes have preconfigured wizards to help you figure it out.
