I knew I had read this somewhere:

Make Magazine Premiere Issue :

Page 103

Make the Cisco Console Adapter.

Required Marterials

Several  RJ45 Ethernet Plugs ( used to make network cables )
UTP 5e Cable
RJ45 Crimping Tool
Two ( maybe 3 ) DB9/Rj45 converters.
One Ethernet coupler.

For the Cisco coupler :

Step 1: Make the First end according to 568b

    Looking left to right with the clip facing down the colours are
    1 white - orange
    2 Orange
    3 White - green
    4 blue
    5 white - blue
    6 green
    7 white - brown
    8 brown

Step 2 Make the Second end :

    1  white -brown
    2  brown
    3  white - green
    4  green
    5  orange
    6  white - orange
    7  white - blue
    8  blue

For the DB9/RJ45 converter


Now you need to make a db9 / rj45 converter, which can usually be 
purchased directly from CPC

You will need to mod these adapters to handle the dte/dcerequirements

so

Step 3 - DB9
 For the pins 2,3,4,7 and 8 from RJ45 to DB9 you do nothing
 For pin 5 on the DB9 it needs to be connected to pins 2 and 6 on the r45
 For pin 1 and 6 on the DB9 you need to connect them to pin 8 on the rj45

( still with me ? )

Now make a plain old Ethernet Cable ( use the 568b instructions for both 
ends as above )

And so we can now build the connector and it goes likes this
