The way PlusNet are going to do it seems pretty fair - a reasonably large 
cap (e.g. around 30 gig) and you can exceed it as much as you want so long 
as you don't do it 2 months running - if you exceed it for 2 months then 
they will throttle your bandwidth down to 128Kbps for the following month. 
All seems quite fair coz it prevents the Kazaa junkies using all the 
bandwidth and if you do need to exceed the cap as a one off you don't end 
up losing your whole internet connection.

Plusnet also have no problems here - I run a web server, VoIP server, etc 
on the end of my DSL.  They were also quite happy to give me a /29 subnet 
for free when I gave them justification for needing it (I know a few ISPs 
do this for free but some charge lots).
