They pretty much end up in the menus; the package itself has to say "put me in 
the menus", but then Debian handles making sure that it ends up in all menus 
(KDE< Gnome, ROX, etc etc). Opera, otoh, isn't in Debian because it's nonfree; 
there may be third-party debs, but third-party debs don't have to follow 
Debian packaging rules...
