Not a good idea. Not only would someone have to be extremely foolish
to sign such a form, it's not in your interests for them to do so. If
you read the fine print of your insurance documents, it almost certainly
says that admitting fault for an accident invalidates the insurance. If
someone does admit fault to you, then you potentially have to pursue
them personally for damages, as their insurance company probably won't
pay up...
