I'm a bit confused between /var/lib/tftpboot and  /tftpboot

Your debugging techniques all seem good to me.

An alarm bell rings when you say 
"I can get /tftpboot/pxelinux.0 using a tftp client from any other
machine on the network."

I think you should try a tftp on another host, and try to
get pxelinux.0  without the prefix.

On our machines:
tftp> get /tftpboot/pxelinux.0
Error code 1: File not found
tftp> get pxelinux.0
Received 11959 bytes in 0.0 seconds


I think you have set the tree up under /var/lib/tftpboot,
rather than the normal /tftpboot
That's OK, but you then have to put /var/lib/tftpboot in the dhcpd.conf
etc.
