Which is expensive to do on a large scale as it has to be implemented on 
the router's acl and that means additional costs for the routers and using 
the time of a network admin/engineer to handle such matters. 

It can be automated which deals with the second problem but not the first.

ISPs tend to run on small margins as it is.
