I'd say forget all the technologies on concentrate first and foremost
on the content, 'information architecture' and the design.
Paper is your friend.
Once you have the look and feel sorted out, then move to the computer
and think about what you need to achieve it.
Using includes in PHP makes sense if the pages will at some pont
become interactive. Frankly there's no difference between SSI and a
PHP include. Does the same job and I would expect with about the same
overhead. Plus if using PHP you can have the included element do
something useful, like indicate the link to the current page, create
breadcrumbs etc etc.
As for tools, for me an old HomeSite on the windows PC and VIM everywhere else.
