Assuming you mean "What determines the hostname my machine offers when
it identifies itself to a remote SMTP server with a HELO/EHLO/LHLO
command", it's just the hostname of your machine. 

Assuming you want to know /which/ of the above commands your machine
will use on connecting, it depends on its own configuration and the
server greeting string - I believe ESMTP should only be attempted to
servers which advertise it as a capability but in practice I think many
do not follow this...
