I use http://www.voiptalk.org and have had no downtime in the last 12  
months.  Links into Asterisk as and IAX2 service too if you want.  Pricing  
seems ok, but I've not investigated further.  You get a free 0870 number  
or can pay a small fee for a regional number if you want to pretend to  
live in Scotland.

They also supply hardware.
