On official company documents I think you need to list the details in the 
way you would on paper - clearly and visibly under orderinary 
circumstances. That being the case I suspect headers are not sufficient on 
such documents. 

Do remember that this only applies as it would on paper - it does not 
include casual and unofficial communications.
