[Gllug] Configuring apple (one-button) mouse

Hello chaps,

How can I configure a one-button apple mouse to accept something like
ctrl+click to behave as middle click?

I don't use gnome / kde or anything with a fancy configuration tool,
so I need to understand how to do this from first principles?

Not really sure where to start, so any pointers would be most helpful.
