Just because neither have been mentioned so far:

  pwsafe

    nice little command line program that is able to populate the paste
    buffer such that you can paste your username and password without
    seeing them

    Arguably makes you safer from shoulder surfing than using nothing,
    as the password they see is the one typed at pwsafe, which is useless
    to them without the password database.  So it's not always true that
    this sort of thing reduces security, depending on your environment.

    Not actually a solution to this problem directly, but if you could
    persuade the site to adopt it would be a solution to the problem in
    general.
