That requirement didn't apply when I signed up with NTL. They keep 
reminding me about their excellent phone and TV service but I haven't 
bitten yet.

I use Telewest. It's £25 a month, but that's for a 4mps connection that, 
touch wood, gives me no problems what so ever and is complelety un-capped.

You do however have to  have their phone and cable tv service as well.
