If you want a truly quiet PC then passive water cooling is by far the
best and these days relatively easy ways to get good end results.
However if that is not on the cards then all of the suggestions made are
the way to go. 

Paul Littlefield's HTPC (construction by myself) has a combination of
FSP Group ZEN fanless PSU, Akasa AK-PAX-2 PaxMate sound proofing, Zalman
CNPS8700-NT CPU heatsink and fan, Fractal Design case fans, a passively
