Whaddaya know, I just crashed Firefox and couldn't kill it.
It absolutely wouldn't budge. I couldn't even close the window, which I 
usually can. kill -9 was as benign as pwd.

Then I ran ps -el as suggested by Matthew and saw the letters usb 
somewhere in the firefox line.

Hmmmm

So I unplugged various things including a digital camera, and as I did 
the Firefox window vanished. I could then restart it without needing a 
reboot.

Weird. But I know what (not) to do now.
