Have you set the extin and extout module options? See the bottom of:

(note that they're listed as snd_extin/snd_extout on that page, but
they're without the snd_ now.) The info is from
/usr/src/linux-2.6/Documentation/sound/alsa/ALSA-Configuration.txt
which is also a good document to read.
