Don't know if it's 100%: get ISPs to refuse to connect spam sources. If 
you can't get on the net because your ISP refuses to connect you because 
you're part of a botnet, that's a pretty good incentive for people to 
run a clean system.

If other ISPs don't do this, you refuse to let them connect to your 
systems. Etc

This, of course, does require cooperation on a massive scale. And 
probably legislation of some sort.
