I've just made a start of using the PEAR LiveUser package in my homebrew CMS
and have been pretty pleased with the flexibility/integratability of it.
http://pear.php.net/package/LiveUser/

I've still to get the LiveUser admin area slotted in, but that's next on the
list. Anybody else used this?

Exactly, site visitors read (with the possibility of a subscription only
area) anything and managers can do anything anywhere within the managers
area.

I was more thinking about different permission levels within the managers
area and the issues involved with making it work properly and safely.
