Opal are spawn of the devil.  They've brought me nothing but misery.

Spitfire rocks!  Spitfire.co.uk.  They're a bit on the expensive side but I get great customer support and they throw in 8 IP addresses for free!  I've currently got 4 lines with them and their VOIP service rocks too.

Also they have no minimum-term contract, you can cancel whenever.
