I've seen this as well, on both the ralink chipset and (much less
frequently) the zd1201 chipset. I've always assumed it was a driver bug.

I haven't been able to tell what my reception quality was like, due to
none of the monitoring tools producing sensible data; these days my
ether is piped, so I'm not using the wireless system any more, so I
can't do any tests.
