FWIW, I've found DokuWiki to be simple and effective.  It's in PHP 
unfortunately, but I've skimmed over the code and it seemed reasonable, 
and in usage it's performed perfectly so far.

Data is saved in plain text files with wiki formatting, so getting data 
in and out of it via other methods is feasible, and no database is required.

I've got a copy of it running at http://wiki.preshweb.co.uk/ for various 
notes I've made (although I've not actually updated it for quite a while).
