Shhh! OK I admit it, there was a config problem and it was nothing to do
with Qmail. (I was running an old version of vmailmgrd and the vdeliver
binary has a -f option which removes this extra non rfc2822 compliant
"from" header before dropping it, which I guess should always be set if
you are using Maildirs. Somewhere in the docs I probably didn't read
something when I set it up originally.)
My POP3 server could probably have been helpful and removed this extra
line before it sent it on to the POP3 client, but I guess it wasn't
expecting to see it.

P.S. I still managed to blame Microsoft to my satisfaction and told the
customer how lucky it was we were using Open Source software and were
able to change things at our end so easily to comply with the MS
softwares stringent requirements. Outlook never complained about this
extra header ;-)
