You could keep the data in Access and use PHP to do the front end.
OpenLink do an ODBC driver that you can download. Alternatively PHP will
cope with the SQL files.
Lots of help and resources for PHP.

If you are putting a proposal together then what about Intersystems (?)
Cache. It is SQL supporting and the most popular amongst Medical apps.
