Depending upon the requirement it may be done over the phone (ie a 
straight forward requirement for a server to provide x y and z) or it may 
require a proper meeting to access the current state and determine what 
the client wants.

Usually the latter is for larger jobs and no, there isn't usually a fee 
for the initial consultation. If further more in depth consultations were 
required there would be fees for those.

Others may work differently.
