Easy up to a point.  Much of the php codebase is really, really ugly.
It takes a lot of discipline to develop anything other than a small
application on PHP, partly because of the work that needs to be done to
overcome the limitations of the language, partly because so there are so
many non-programmers (people whose only other experience - if any - in
coding is with VB) coding in PHP and churning out crap.

Horde is an example of what can be done if that discipline is imposed.
SquirrelMail is sorely in need of that discipline.  They're trying to
get there but I'm not sure if they are moving forewards or backwards.
When core developers do things like duplicating an entire long smtp
function, changing one line in the second implementation and giving it a
slightly different name, the second function to be called in some rare
circumstances, then you have serious problems.

We use SquirrelMail but if I could do it over again I'd probably choose
something else.
