I saw an Apache config fragment once that only allowed access to image
filetypes if they were referred from a page on the same server.

Sorry it's a bit vague but a quick google finds this for example:

http://apache-server.com/tutorials/ATimage-theft.html
http://www.phpfreaks.com/apache_manual/page/env.html

Stops direct linking to images but that's about all, might be what you want.
