[Gllug] How can Spam bypass SpamAssassin entirely?

Hi all,

I just had a piece of Spam drop into my mailbox, and it was such obvious
Spam I thought I'd take a look at the source to see how it had defeated
both SpamAssassin and Bogofilter.  The really odd thing is - it doesn't
seem to have been processed by SA at all.  None of the usual SA headers
has been added.

SA is invoked from my .procmailrc file.  How could an incoming e-mail
avoid SA entirely like this?
