Haven't used it for quite a long time because I moved to a 2-room flat
with the computer in the same room as the telly, so we just watch stuff
on the computer monitor.  (Why I offered it for loan)

Last time I used it--and this was a couple of years ago--mplayer had
solved all the sync problems with one requirement: the user running it
has to have RTC access.  From memory, this meant root.

If you run vdr[1], there is a plugin to get the OSD to work through the
DXR3.  VDR is designed to run on "Full" DVB cards, which have an MPEG
decoder.  Other DVB stuff doesn't, so the DXR3 can be used.

There was someone floating around who was writing a more generic OSD
thing for the DXR3.  I'd be surprised if that hasn't been plugged into
other stuff like MythTV.
