Many years ago I worked somewhere that did this (now long-bankrupt).

There was a two-pronged approach:

- Firstly, whenever you wrote or changed any code, ask yourself 'is this
  a bug?' if it is a bug, fix it. This astonishing idea should drive the
  bug rate down to zero.

- If by some chance it didn't, all bugs were redesignated as 'issues'.

Then marketing went out and plugged the product as bug-free. Oddly the
(soon to be ex-)customer base didn't entirely agree with this claim.
