This is from a Ubuntu box with a working eth0:

sudo grep -r eth0 /etc/

/etc/initramfs-tools/initramfs.conf:# Specify the network interface,
like eth0
/etc/initramfs-tools/initramfs.conf:DEVICE=eth0
/etc/dhcp3/dhclient.conf:#  interface "eth0";
/etc/dhcp3/dhclient.conf:#  interface "eth0";
/etc/udev/rules.d/70-persistent-net.rules:SUBSYSTEM=="net",
DRIVERS=="?*", ATTRS{address}=="00:19:66:21:c5:83", ATTR{type}=="1",
NAME="eth0"

Maybe give you a clue to what is missing?
