I'm sure everyone has their favorites for various reasons. All I will
say is that I use my connection for similar things to those you describe
and I have used demon and also black cat networks (my current ISP) I
have been happy with both.  With black cat you get the added bonus of
knowing it is run by some cool linux guys who provide good support and
you also get ipv6 access thrown in as an extra if you are interested in
it.

They are not the cheapest around but fit within your stated price.

As for modems, I will let someone else advise you on that.
