Actually scratch that, I just noticed my Mac had defaulted to the US 
keyboard for some bizarre reason so the pound and the pound (err... hash.. 
if you must) were reversed, but under the British keyboard map I can 
confirm that Alt-3 does work fine for me in VIM insert mode!  Running 
10.4.10 here, nothing special, no fink, etc.
