You need RRDTool - If you want to make life really easy, use Cacti to 
manage it and display it for you.

An example setup (mine) is here: https://heliosuk.net/cacti
(username: guest, password: guest)

This setup picks up SNMP stats from my various systems, and displays 
them for you.

I wholeheartedily recommend.
