A few more things for me to try in my pursuit of a stable 8128 Kbps :)

In addition to Chris's excellent suggestions, disconnection the ring wire to
extensions helped my connection a little with the standard NTE5 faceplate
and wiring to going to the extension over CAT6.

of this - on my extension 4 isn't connected either - just 2 & 5.
