Thanks for the tip.  I have already been there.  I clear out the cache
manually and use different browsers to make sure.

I can't help you with .htaccess, but beware of cacheing by the browsers. I
have known web developers tear their hair out becase changes they do don't
seem to have any effect, only to find that a previous response has been
cached by the browser and it's not even going back to the web server again.
