For a pick-up-and-go guide I found 'PHP for the World Wide Web' by Larry 
Ullman quite useful.  I think it's published by Peachpit Press.

It does miss some stuff out(although admittedly I was using PostgreSQL 
rather than mySQL), but it's easy to fill that in from the online docs 
on the PHP website.
