I have to say that this all sounds horribly familiar.  I had similar
problems when one of my hard drives was starting to pack up and die. 
Before you do anything else can you get your drive manufacturers
diagnostic software?  I'd be inclined to run that using the
non-destructive tests.  They usually have a facility to rebuild the
MBR aswell.  But, it will be rebuilt to as new unformatted I believe
so be careful.
