We do? Where?

'Supposed' being the operative word

Because that was the hospital he chose, for whatever reasons he had. Would you
suggest, if I were to make a _personal_ charitable donation I should spread
it evenly around, or give to my one chosen organisation?

But the exception was made because when the bequest was made the law held that
the rights would be property of the hospital in perpetuity. The 1988 act
changed that, so an exception is made to maintain the status quo. I can
imagine the hospital trusties complaining about the (marginal) loss of
income.
