I greylist. I get effectively  _zero_ spam[0] and the very very few 
viruses/worms and spams that get past that get zapped by clamav and 
spamassassin.

[0]  I'm up to my second spam message since the new year, and that was 
UCE from a company who then honoured my unsubscribe request.
