I've just got back from a conference for a VoIP system supplier, and I
got the feeling that these guys really care about what they are doing
and even have a clue.

For a linux based commercial VoIP system you should check out:

http://www.splicecom.com

They have just released SIP trunk capabilities, the systems based on a
load of 1U modules that easily replace existing phone systems without to
much pain, in fact I'm running one with around 100 extensions in the
school that I work in.

Give it a look if Avaya and Cisco aren't doing what you want them to do.
