We use Mindtouch Wiki www.mindtouch.com. Its a commercial Open Source 
project , so the core is open source, and some (enterprise) features are 
in the commercial version.

The WYSIWYG editor is excellent and wiki pages are based on XHTML. 

I havent skinned it, but it doesnt seem too bad to skin it.
