The Template is probably the best bet then.  People could download, add 
the info, save and either print their own copies or email directly to 
you for printing.

With the first link which opens in Google Docs, half the logo is missing.

As far as I can remember it covers all we discussed but I'll wait for 
Pete to confirm.
