It sounds to me as though a Dynamic QR Code is purely one that points to 
a redirection URL so you can swap the destination.

So one way is to use a short url/redirection service that allows you to 
edit the target.  There are a few of these (although many of the bigger 
ones do not allow editing of the target as its sometimes used as a bait 
and switch phishing approach).  Possible services include:-
	http://www.qrstuff.com/blog/2012/08/12/dynamic-qr-codes
	http://at5.us/
	https://www.joturl.com/
	http://notlong.com/
	http://snipurl.com/

Not tried any of these

Alternatively, a couple of lines of javascript, backed maybe by a HTML 
meta tag if things support that, would give redirection from a page 
without requiring access to the webserver.
