You're not quite in cloud cuckoo land..... try looking a the open h323 pages (http://www.openh323.org) last time I looked they were cracking on.

The advantage of using h323 is that it should interface nicely to the rest of the world.

As to creating a soundcard to phone connection.... they do have some gateways (if they do this sort of thing) but you might need to code something up yourself.
